Registrar Forms
The Registrar's Office is now hosting important forms online at SLATE FOR STUDENTS, central online location for academic forms and resources. You will now be able to submit requests to the Registrar’s Office, obtain the approval of your advisers and academic departments, upload additional supporting information, and track the status of your pending requests through the Slate for Students online portal. This system will also allow you to update your personal profile information, such as your address, phone and directory preferences, as well as find the contact information for your advisers.

Signatures and Recommendation Letters
Barnard forms often require your advisor’s, an instructor’s or the chair’s approval. If you intend to submit a petition to a Barnard committee, make sure that you contact the department ahead of time.
- Let our departmental administrator, Sondra Phifer, know your request and inquire about the faculty member’s office hours or his/her online availability via Zoom.
- If you need our support for an application, we are happy to provide your with an instructor’s evaluation or a letter of recommendation. We advise you to contact the faculty member at least two weeks before the due dates for the letters. Please collect the required documents and submit them either as a hardcopy or via email in an attachment to your request.
- If you need to obtain the department chair's signature or a recommendation, please make sure that you put in your request before the specified deadline.
